After hospital discharge: What a 30-day trial suggests about pharmacist follow-up for seniors

Older hospitalized patients who struggled to take their medications correctly were 10% less likely to need to return to the hospital if they had help from a pharmacist at the time of discharge, according to a new multisite clinical trial based at Cedars-Sinai. But the findings, published in JAMA Network Open, showed that the extra help did not significantly reduce unplanned hospital visits for the rest of people age 55 and older.
